Statement of the Job
Coordinate the completion of resident assessments and care plans in a timely manner for submission within State, Federal and accreditation regulations and standards.
Duties of the Job
Work to lead a positive, cohesive team, providing motivation for the best quality care possible while demonstrating respect and a positive attitude towards associates, residents and family members.
Complete MDS, RAPS, and Care Plans in an accurate and timely manner.
Ensure that transmittal is completed on a routine/timely schedule per State and Federal regulations.
Supervise staff for completeness and quality of work and compliance with facility policy. Address problem areas with administration for appropriate corrective action.
Maintain schedule for PCP meetings.
Train other personnel on correct procedures for completion of the RAI Process.
Perform necessary audits as requested.
Conduct care plan meetings with Interdisciplinary team and family members.
Maintain appropriate contact with families utilizing positive customer relations.
Coordinate efforts with other departments, including: Social Services,
Activities, Nursing, Rehab and Dietary regarding: RAI/PCP.
Print monthly QI reports and distribute.
Comply with nursing department and facility policies and procedures.
Evaluate the performance of unit personnel in accurate and timely manner.
Make rounds, assess residents and monitor 24-hour reports to ensure accuracy of work.
Monitor for significant change. Track re-entry/discharge.
Maintain security/confidentiality of RAI/PCP.
Other duties as assigned by DON/Administrator.
Job Qualifications
Education/
Experience:
Licensed RN with three (3)
years experience.
One (1)
years experience completing MDS/Care plans.
Demonstrated understanding of State, Federal and Accreditation regulations and standards.
Proven ability in communication, supervision, assessment and follow-up skills.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
